Benzodiazepine tranquilizers & Tramadol

Benzodiazepines (BZDs) are a class of psychoactive drugs known for their depressant effects on the central nervous system (CNS). BZDs primarily exert their effects by enhancing the activity of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA). They bind to specific sites on the GABA-A receptor, a type of receptor in the brain that responds to GABA. When BZDs bind to these sites, they increase the efficiency of GABA binding to its receptors. This action enhances the inhibitory effects of GABA on neuronal activity, leading to sedation, muscle relaxation, reduced anxiety, and anticonvulsant effects.


Examples of Benzodiazepines:
  • Diazepam (Valium): Often prescribed for anxiety disorders, muscle spasms, and seizures. It can also be used as a sedative before certain medical procedures.
  • Lorazepam (Ativan): Commonly used to treat anxiety disorders. It may also be used for insomnia, seizures, and as a sedative before surgery or other medical procedures.
  • Alprazolam (Xanax): Primarily used for the treatment of anxiety and panic disorders.
  • Clonazepam (Klonopin): Prescribed for panic disorders and certain types of seizures.
  • Temazepam (Restoril): Mainly used for the short-term treatment of insomnia.


Tramadol is a centrally-acting analgesic that is commonly prescribed for the treatment of moderate to severe pain. Its mechanism of action is unique and multifaceted. Tramadol acts as an agonist of the μ-opioid receptors in the CNS. This means it binds to these receptors and activates them, leading to analgesic effects similar to other opioids. Apart from its opioid activity, tramadol also inhibits the reuptake of two important neurotransmitters: norepinephrine and serotonin. This action can modulate pain signals in the CNS, contributing to its analgesic effects.

The combination of μ-opioid receptor agonism and monoamine reuptake inhibition gives tramadol its unique dual mechanism of action, making it effective in treating various types of pain, including neuropathic pain.

Tramadol is metabolized in the liver to produce an active metabolite called O-desmethyltramadol (M1). This metabolite has a higher affinity for the μ-opioid receptor than tramadol itself and contributes significantly to tramadol's overall analgesic effect.


When BZDs and Tramadol are taken together, there is a potential for additive CNS and respiratory depression. This is because both drugs depress the CNS, albeit through different mechanisms. Studies suggest that tramadol-induced seizures, which can occur in overdose situations, are mainly related to a GABAergic pathway, and not necessarily serotoninergic pathways as previously thought.

Common side effects include nausea, vomiting, dizziness, constipation, and CNS depression. Serious side effects can include seizures, serotonin syndrome, respiratory depression, and overdose, especially when taken with other CNS depressants like benzodiazepines. The combined use of tramadol and benzodiazepines can lead to enhanced CNS and respiratory depression, increasing the risk of overdose and death. There is also an increased risk of seizures, especially in the context of tramadol overdose.

I had seizure and the worst nausea I've ever felt in my life.
2 days of constant vomiting.
Somehow I survived.
