Sadly both the original Aminorex and it's RC derivatives (4,4-DMAR, 4-MAR and it's 4-halogenated versions) are notorious for being responsible for many deaths even among casual users. Especially repeated and prolonged use seems to be dangerous. This isn't some kind of "maybe" danger, it is definitely known to cause severe lung complications that more often than not are fatal.
I cite a reddit comment about this:
4-Methylaminorex, like aminorex, seems to cause pulmonary arterial hypertension (PAH). Aminorex was a prescription drug that got pulled from the market two years after its introduction because of this. The aminorex-related cases of PAH were often severe: half of patients with aminorex-related PAH were dead within 10 years from heart failure.
This is both the reason for MAR's removal from the market as a pharmaceutical and also why none of it's analogues became RC shootingstars like many other cathinones or amphetamines, although it was said to have partly superior effects to them.

I throw another substance into the ring that funny enough is made from the same precursor as 4-MAR:
Phenmetrazine
It is said to be a clean, very euphoric upper. This whole class of stimulant drugs is often overlooked and somehow got forgotten.
